U.S. DEPARTMENT OF TRANSPORTATION SERVICE ANIMAL AIR TRANSPORTATION FORM
Beginning March 1st 2021 individuals traveling by air with a Service Animal must fill out a form provided by The Department of Transportation.
This form asks basic questions about the handler and their Service Animal but one question is asked that many people do not know how to answer.

The question implies the Service Animal was required to be trained by an professional organization.
The Americans with Disabilities Act does not require Service Animals to be professionally trained. Individuals with disabilities have the right to train a Service Dog themselves and are not required to use a professional Service Dog trainer or training program.
Dogs can be self-trained to assist a person for many common types of disabilities. These types of Service Dogs may include Hearing Dogs, Seizure Response, Diabetic Alert and PTSD Service Dogs. Service Dogs help promote independence, security and confidence to their handlers.
